Complete State Application
  1. Click on                          to get started on an application with the Hawaii Dept of Health

  2. Under "Enter your physician's/APRN's first & last name" add: Abir Amirdash (APRN license 2122)

  3. You have the option to add a caregiver and/or a grow site. (please refer to FAQ section for more information)

  4. Upload a valid picture ID (A US state ID, driver’s license, or passport)

  5. Pay the DOH fee of $38.50 (non refundable State fee) and submit your application.

  6. If you require assistance with completing your application, contact us during business hours.

Get Evaluated

If you are new to our practice, first time evaluations and are in person. Renewals for our existing patients are telehealth. Evaluation takes 15-30 minutes, depending on your condition. Abby, the APRN, will discuss your medical condition and you will be provided with dose guidance and education about use and rules and regulations.

Processing time is determined by the state. It takes an average of 1-5 business days.

Approval is guaranteed. Once your card is processed by the state, you will receive an email.

To access your card, login to the state website and download a digital copy of card on your smart device or print a hardcopy.  Cards are valid for one year with initial certification. Renewals are for 1 or 2 years, fees vary.
