Complete Your In-State Application
Start and complete your application on the Hawai‘i Department of Health (DOH) website. Save your login info , you’ll need it to access and download your 329 card once approved.
What You’ll Need:

Certifying Provider’s Name: Abir Amirdash, APRN

Unexpired Photo ID: Accepted forms include any U.S. state ID, driver’s license, or passport

State Fee: $38.50 payable online to the state directly.​

What to Expect During Your Evaluation

New patients: whether you are getting a new card or renwing your card, all evaluations are telemedicine visits using a HIPAA compliant secure platform. Your visits can be a Video visit or telephone call.

Returning patients: Renewal evaluations are completed as a telephone call.​
Your appointment will typically last 15-30 minutes, depending on your type of your appointment.

Your 329 Digital Card:
The processing time for your medical cannabis card is determined by the Hawai‘i Department of Health and typically takes 1 to 3 business days.
✅ Approval is guaranteed once your application is submitted correctly. You’ll receive an email notification from the DOH once your card is approved to download.
To access your card:

Log in to the HI state medical cannabis registration with your credentials

Download a digital copy to your device or Print a hard copy

Cards are valid for one year with initial certification.
Renewals can be completed annually or every two years and require an updated evaluation.
