Complete a State Application
Start and complete your application on the Hawai‘i Department of Health (DOH) website. Save your login info , you’ll need it to access and download your 329 card once approved.
What You’ll Need:

Certifying Provider’s Name: Abir Amirdash, APRN

Unexpired Photo ID: Accepted forms include any U.S. state ID, driver’s license, or passport

State Fee: $38.50 payable online to the state directly.​

What to Expect During Your Evaluation

New patients: Evaluations are conducted via telehealth or in person.

Returning patients: Renewal evaluations are completed as a telephone call.​
Your appointment will typically last 20–40 minutes, depending on your condition.
During your evaluation, I will:

Review your medical history

Check for medication interactions

Provide personalized recommendations and dosing guidance tailored to your needs.

Information on Hawai‘i’s cannabis laws and regulations.

Your care doesn’t stop after certification. Each patient is entitled to a complimentary 10 min follow-up within a month, included with your visit, for dosing guidance or side effect management. Whether you're adjusting your regimen or have questions about your response to cannabis.

Receiving Your 329 Card
The processing time for your medical cannabis card is determined by the Hawai‘i Department of Health and typically takes 1 to 3 business days.
✅ Approval is guaranteed once your application is submitted correctly. You’ll receive an email notification from the DOH once your card is processed.

Cards are valid for one year with initial certification.
Renewals can be completed annually or every two years and will require an updated evaluation.
