The process has two parts to obtain your 329 card/Medical Marijuana Card:
Consultation Evaluation: Book an appointment. There are 2 different types of appointments.
329 Evaluations/ Renewals- streamlined for patients familiar with the process. Requires a completed patient state application & completion of clinic forms prior to the consult appointment. Includes a review of your medical history, determination of eligibility, submission of your completed application to the state & information about program rules & regulations. Pricing is $105 for 1 year & $ 210 for 2 years for eligible returning patients.
Cannabis medicine consults combined with 329 evaluations/renewals: includes a review of your medical history, determination of eligibility, evaluation of any drug interactions & cannabis, and a personalized cannabis dosing reccomendations tailored to your health needs. Best for patients new to Cannabis or patients who need help with their dosing.
Pricing: $150 plus added $25 for new patients. Returning medical cannabis consult patients can elect for a follow up $100 with added 329 evaluations/renewal for $ 75 for 1 year & $125 for 2 year renewals.
All patient appointments are telemedicine. You can choose from a video call using a HIPAA compliant platform or as a secure telephone call. Once you book your appointment, you will receive information with the link to your appointment.
2. Completeing a State Application: Complete your patient application with the Hawaii State Department of Health & pay the state Fee. For Certifying provider - add Abir Amirdash, APRN license 2122. Assistance with your application is available during your evaluation for an additional fee.
$20 for assistance with application completion.
$40 for assistance with application completion with caregivers &/or added grow sites
Full payment for your consultation is due at the time of service . Consult cost is separate from your application state fees.

ALL Appointments are done as Telehealth using a HIPAA-secure link sent prior to your visit. You can also choose for a telephone appointment.
Returning patients renewals: Completed as a telephone call.
Cannabis medicine Dosing: Can be either a video or a telephone call.

Alongside Cannabis Medicine consults & 329 certifications, I offer cannabis speaking engagements, health & wellness coaching, & Reiki services. For non cannabis related health concerns, Abir is available on HMSA online Care platform for episodic visits- usually covered by your insurance provider.
Any cannabis related consults/visits are not covered by any medical insurance due to federal cannabis status.
Pricing:
329 evaluations/ renewals- fast track: $105 for 1 year certificate, Renewals for 2 years is $210
Cannabis medicine Consult + 329 evaluation: $150+ $25 for 1 year
Cannabis medicine consult follow up/ Dosing guidance: $100 Follow up consult for patients requiring extra guidance using cannabis, tailored to your needs. For existing patients. Include a 329 Renewal for 1 year for $75 ( $175 total ) or 2 years for $125 ( total $225)
Health & wellness Coaching: For help managing your health. Pay per session, from $225 to $275
Cannabis Medicine Speaking engagements: Available for community events & educational support for healthcare colleagues - contact for more info.
Additional Fees:
An additional $20 fee will be added for assistance with patient state applications.
An additional $40 fee will be added for assistance with patient state applications that include caregivers &/or grow sites
Please note the state application fee is an independent & separate cost from your evaluation cost.
Payment options: We accept all debit & credit cards, Venmo, & Cash app. Secure payment processing through Square. Customer payment information is NEVER stored on file.

In Hawai‘i, only medical cannabis cardholders can access dispensaries.
A caregiver is someone you designate to attend dispensaries on your behalf or accompany you, purchase your medicine, and legally carry it for you. A caregiver can also cultivate cannabis for qualifying patients.
We especially encourage patients who are disabled or undergoing extensive treatments to assign a caregiver when completing their state application to ensure safe and convenient access to their medicine.
Per the state: Primary caregiver” means a person eighteen years of age or older, other than the qualifying patient and the qualifying patient’s physician or advanced practice registered nurse, who has agreed to undertake responsibility for managing the well-being of the qualifying patient with respect to the medical use of cannabis. In the case of a minor or an adult lacking legal capacity, the primary caregiver shall be a parent, guardian, or person having legal custody. for more info contact OMCCR directly at 808-586-4400

Patients have the choice to cultivate up to 10 cannabis plants & limited to five cards per location with the exception of inhabited islands without a dispensary- currently: Moloka'i, Lanai & of course Ni‘ihau! your plants must be tagged. Tagging & labeling your plants.
Patients opting to grow should consider adding this option initially to avoid additional fees.
Medical cannabis may be grown at one of the following locations and that location must be identified on your 329 Card:
the residence of the qualifying patient, or
the residence of the designated primary caregiver, or
another site that is either owned or controlled by the qualifying patient or the designated primary caregiver.

All use of medical cannabis must be on private proper. Do not use cannabis in your car while on the road, at work, at school or on school grounds, at the beach, at a park, on hiking trails, on a bus, or in any other public space. It is illegal to use or possess cannabis on any federally owned property such as military installations and national parks.

Although medical cannabis is legal in HI, driving under the influence of cannabis is illegal and can be dangerous.

When transporting medical cannabis away from home, ensure it is stored in a sealed container and kept out of public view. You are able to carry up to 4 ounces for personal use.
It is important to note that flying, including inter-island travel, with any cannabis remains illegal under federal regulations. Even with a medical certification, patients are not protected when transporting medical cannabis inter-island, between states, or internationally.
